Hospice Admissions Nurse
The Hospice Admissions Nurse is a Registered Nurse responsible for managing all aspects of the patient's intake process.
After Hours Nurse
The After Hours Nurse will serve as a member of the After Hours team. The After Hours Nurse is responsible for ensuring that patient care and services are delivered, coordinated and managed appropriately after hours. The After Hours Nurse will take calls after hours and make patient visits as needed. Prior hospice experience is preferred.
Hospice Registered Nurse
The Hospice Registered Nurse is responsible for ensuring that patient care is coordinated and managed appropriately. The Hospice Registered Nurse is responsible for ensuring that care and services are delivered appropriately and for the supervision of other members of the IDG team.
Licensed Practical/Vocational Nurse
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Certified Home Health Aide
The CHHA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The CHHA is responsible for observing the patient, reporting these observations and documenting observations and care performed.
