Human Resources is responsible for handling duties and functions related to recruitment, benefits, training and development, and employee relations. Responsibilities will include performing recruitment activities; on boarding and off boarding; assisting in the development and maintenance of job descriptions and policies and procedures; benefit enrollment, personnel data entry, file maintenance, compiling and generating HR related reports and general HR administrative support activities.
Hospice Social Worker
The Hospice Social Worker is responsible for providing psychosocial assessments and evaluation for patients and their families/caregivers, planning adn implementing interventions as need, maintaining clinical records on all patients regarding social work needs, developing the social work componenet of the plan of care for each assigned patient, and serving as a liaison between teh patient and community resources. Qualifications for this position include education of a Masters of Social Work or higher and a minimum of one year of hospice experience.
Hospice Registered Nurse
The Hospice Registered Nurse is responsible for ensuring that patient care is coordinated and managed appropriately. The Hospice Registered Nurse is responsible for ensuring that care and services are delivered appropriately and for the supervision of other members of the IDG team.
Licensed Practical/Vocational Nurse
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Certified Home Health Aide
The CHHA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The CHHA is responsible for observing the patient, reporting these observations and documenting observations and care performed.
