Hospice Services

One in three Americans is caring for a loved one with a serious illness. They find themselves looking for answers to important questions and making challenging decisions every day. It’s not something anyone should have to go through alone. Apreva Hospice is there to help families when they need it most.

Apreva cares for the terminally ill and helps patients live their final days to the fullest, as free as possible from disabling pain and mental anguish – usually in the setting of their own home, or the home of their choice.

Hospice care embraces the patient as a whole, encompassing their physical, social, emotional and spiritual needs. We do this in numerous ways, including:

  • Enhancing the quality of a patient’s life
  • End-stage symptom management; such as pain, agitation, nausea, trouble breathing, secretions, or difficulty swallowing
  • Interdisciplinary team; physicians, nurses, home health aides, social workers, spiritual counselors, and volunteers
  • Preserving the patient’s dignity and sense of peace
  • Encouraging the patient and family to guide the plan of care
  • Addressing anticipatory grief
  • Bereavement services to aid in the mourning, grieving, and bereavement process
  • Educating community and healthcare professionals regarding symptom management and the unique needs of terminally ill patients and their loved ones

Levels of Care

Apreva provides the level of care that is most appropriate to meet your individual needs.

Routine Home Care

Provided to individuals in the comfort of their home, whether the home is a private residence, a long term care facility, or an assisting living community.

Inpatient Care

Short term inpatient care at a hospice-approved facility.  Patients receive intense palliative treatments to address pain and symptom management. Once symptoms are controlled, the patient returns to their home.

Continuous Care

This level of care is medically necessary care for patients who may be experiencing a medical crisis requiring more intense pain or symptom management for short term extra support.

Respite Care

As needed for family members or caregiver relief up to 5 days on an occasional basis at a hospice approved facility.

Your Care Team

Apreva’s specially trained interdisciplinary team of healthcare professionals and volunteers provide the latest and most innovative approach to address all aspects of care, allowing the patient and family to focus on maintaining quality of life. 

We have the following care providers on our team:

Hospice physicians are specially trained and knowledgeable in the most effective and appropriate treatment of end of life symptoms. The Apreva Hospice physician will coordinate with the patient’s primary care physician to assist in providing the best route of care.  This allows the patients continued continuity of care.

Having a trustworthy nurse provide care is crucial. The Apreva Hospice nurses provide knowledgeable, competent, and compassionate care. They specialize in symptom management, caregiver education, and palliative treatment options. Nurses are available to visit 24 hours a day and diligently work with physicians to oversee care.

An Apreva Hospice Certified Home Health Aide (CHHA) offers personal care to the patient. An example of CHHA task’s might include: personal grooming, hygiene, assistance dressing, toileting, incontinent care, repositioning bed bound patients, assistance with walking, linen changes, a shower, tub bath or bed bath.

In addition to nurses, physicians, and certified home health aides, Apreva Hospice provides access to Social Workers, emotional and spiritual support, and volunteers. There is also a myriad of other palliative therapies available such as music therapy, aroma therapy, gentle massage, light housekeeping and grief support, just to name a few.
